If your dentist has told you, “I don’t think another filling is going to be enough,” you may be wondering why. If the tooth already has a filling, why can’t the old material simply be removed and replaced with a new white filling?

The answer comes down to something surprisingly simple: how much healthy tooth structure is left and whether that structure can withstand normal chewing forces.

A composite resin white filling is an excellent way to repair many cavities. It is conservative, tooth-coloured and can restore a tooth when enough natural structure remains to support it. But a filling cannot replace every part of a tooth’s mechanical strength. When a tooth has lost too much structure through decay, repeated fillings, cracks, wear or fracture, a permanent restoration that protects the remaining tooth may be the better choice.

So, when should you get a dental crown vs. a white filling? The decision is not based on the size of the cavity alone. Your dentist considers the remaining tooth structure, the location of the damage, the condition of the cusps, your bite and whether cracks are present.

The Threshold of Tooth Preservation: Composite vs. Full Coverage

The goal of restorative dentistry is not simply to “fill the hole.” It is to restore a tooth while preserving as much healthy natural structure as possible.

For a relatively small cavity, composite resin can work extremely well. The decayed portion is removed, the tooth is prepared and the composite is bonded into the prepared area. Because the filling is bonded directly to the tooth, it can provide an attractive and conservative restoration.

The situation changes when the restoration becomes very large or when the tooth itself has been structurally compromised.

Imagine a tooth as a small engineered structure. Enamel and dentin work together to distribute forces. Remove a significant amount of that structure and the remaining walls and cusps have to absorb more stress. A filling can occupy the missing space, but it does not necessarily recreate the original architecture of the tooth.

That is why your dentist may recommend a crown even when you have no pain.

A crown is designed to cover and protect the remaining visible tooth structure. The Ontario Dental Association describes crowns as restorations used to cover teeth that have been broken, damaged by decay or otherwise compromised.

The Limitations of Large Composite White Fillings

  1. The "50 Percent Rule" of Natural Tooth Structure

You may hear people talk about a “50 percent rule”—the idea that if more than half of the natural tooth remains, a filling is appropriate, while less than half means a crown is required.

There is no universal dental rule, however, that says exactly 50 percent of a tooth must remain for a filling to be successful.

Instead, dentists evaluate the quality and configuration of the remaining tooth structure. Two teeth could each have 50 percent of their original structure remaining but have very different levels of stability.

For example, one tooth might retain strong, intact cusps and healthy walls. Another might have lost both marginal ridges, have a large filling extending across multiple surfaces and contain a crack running toward a cusp.

Those teeth do not have the same prognosis.

Your dentist may consider factors including:

  • How many surfaces of the tooth are involved
  • Whether one or more cusps have been weakened
  • Whether the marginal ridges remain intact
  • The thickness and height of the remaining walls
  • Whether cracks are present
  • How much previous filling material has already been removed
  • The location of the tooth
  • Your individual bite and chewing forces
  • Whether you clench or grind your teeth
  • Whether the tooth has undergone root canal treatment

This is why the question is better phrased as “Is there enough structurally sound tooth left to support a filling?” rather than simply “How big is the cavity?”

  1. Cusp Fractures and Chew-Force Distribution

The cusps are the raised portions of a back tooth that help it break down food. They also play an important role in distributing chewing forces.

Posterior teeth experience substantially greater forces than front teeth. Research measuring biting and chewing forces has demonstrated that forces are greater in the posterior regions of the mouth, with maximal habitual bite forces measured in the hundreds of Newtons.

When a large filling removes or separates the structures that normally help distribute those forces, stress can become concentrated in the remaining tooth walls and cusps.

This is one reason a tooth with a large existing restoration can eventually develop a cusp fracture.

You might notice:

  • A piece of the tooth suddenly breaking while eating
  • Pain when biting down or releasing your bite
  • Sensitivity to cold
  • A sharp edge on the tooth
  • A filling that repeatedly breaks or falls out
  • Intermittent pain that is difficult to pinpoint

How a Dental Crown Shields a Vulnerable Tooth Structure

A dental crown works differently from a conventional filling.

Rather than occupying only the cavity, a crown covers the prepared tooth and is designed to protect the remaining structure from damaging forces. The preparation is carefully shaped so the crown can fit over the tooth and function with your bite.

Crowns may be recommended for teeth that are cracked, chipped, have large fillings or have undergone root canal treatment.

Think of the difference this way: A filling repairs a missing section; a crown protects a weakened structure.

A crown does require tooth preparation, meaning some additional tooth structure must be removed to create the space and shape needed for the restoration. That is why a crown should not automatically be considered the “better” treatment for every cavity. When a conservative filling can predictably restore a healthy tooth, preserving natural structure is valuable.

But when the tooth has crossed the threshold where a filling cannot adequately protect it, choosing the more conservative option simply because it involves less treatment may not actually be the most conservative choice in the long term.

The Hidden Risk of Delaying a Crown: Root Fractures and Extractions

One of the biggest concerns with a structurally compromised tooth is that the next fracture may be much more serious than a chipped cusp.

If a crack extends deeply into the tooth and eventually reaches the root, the tooth may become difficult—or sometimes impossible—to restore. In severe cases, extraction can become necessary.

That does not mean every tooth that needs a crown will fracture if you wait. Nor does it mean that a crown guarantees a tooth will never break. Rather, the concern is that once a tooth has significantly weakened, continued chewing forces can turn a manageable structural problem into a much larger one.

This is particularly important for teeth that have already undergone root canal treatment.

One common explanation is that root canal-treated teeth “dry out” and become brittle. That is an oversimplification. The greater issue is usually the loss of tooth structure caused by the original decay, fracture, trauma and the access required for root canal treatment. Evidence suggests that cuspal coverage can improve the fracture resistance and clinical survival of many structurally compromised posterior teeth, particularly when substantial tooth structure has been lost.

Importantly, research also shows that not every root canal-treated tooth automatically requires a full crown. Teeth with relatively limited structural loss may sometimes be successfully restored with bonded composite or other conservative approaches. The amount and configuration of remaining tooth structure matter.

In other words, the decision should be individualized—not automatic.

What to Expect: Transitioning Your Smile Seamlessly

If your dentist recommends replacing a large filling with a crown, the first step is an assessment of the tooth.

Your dentist may:

  1. Examine the remaining tooth structure to determine whether the walls and cusps are strong enough to support a filling.
  2. Look for cracks or fractures, including areas that may not be obvious during a routine visual examination.
  3. Evaluate your bite to determine where chewing forces are concentrated.
  4. Take dental X-rays when appropriate to evaluate decay, the roots and surrounding bone.
  5. Remove existing decay or failing restoration material to determine how much healthy tooth remains.
  6. Assess whether additional treatment is necessary, such as a core restoration or root canal treatment.
  7. Prepare the tooth so there is adequate space and support for the crown.
  8. Place a temporary restoration when required while the permanent crown is fabricated.
  9. Fit and cement the permanent crown, checking its appearance, contacts and bite.

The result is intended to look and function like a natural tooth—not like a large piece of dental work.

Can a filling be replaced with another filling if it drops out

Sometimes, yes—but not always.

If a filling falls out and there is still sufficient healthy tooth structure, your dentist may be able to replace it with another filling. However, if the filling was already large, the tooth has developed a crack, additional decay is present or more tooth structure has fractured away, another filling may no longer provide adequate structural support.

When you're deciding when to get a dental crown vs. white filling, the most important factor is not whether a filling can physically fit into the cavity. It is whether the remaining tooth can reliably support that filling under years of chewing forces.

A white filling is often an excellent choice when enough healthy tooth structure remains.
